Health Specialist, 30 hrs/week, Excellent Benefits.
The Health Specialist will provide prevention education and light case management for Deaf, DeafBlind, Hard of Hearing and Late-Deafened individuals living with obesity, diabetes, dental problems, cancer, and other health related conditions.
- Conduct 1:1 and group public health awareness and prevention education activities. - Conduct weekly fitness classes and cooking classes. - Outreach to Deaf, DeafBlind, Hard of Hearing and Late-Deafened people and encourage healthy choices. - Provide case management services including facilitating assessment of consumers' health needs, development and monitoring of individualized service plans, coordinating referrals for a wide range of support services, and provision of support related to utilizing resources for self-help and health maintenance. - Collaborate with health and medical service providers, to promote and advocate for accessible service options. - Provide peer support services including developing and operating regular peer support activities and groups. - Prepare written and statistical reports on activities and progress. - Develop public health brochures, posters and write articles. - Co-coordinate annual health fair and AIDS Walk events. - Be available to work some evenings and weekend hours when needed.
Qualifications: Bachelor's degree in Human Services area. Fluency in ASL. Demonstrated knowledge of deafness, at least three years in a related field. Health care related experiences and knowledge of public health issues required. Experience in direct services preferred. Ability to prepare and facilitate trainings and presentations. Computer literate.
